First Visit & Paperwork

Brief Paperwork

Upon entering our Mandeville chiropractic office, we have some brief paperwork for you to fill out. The paperwork provides us with general information about yourself and your condition. The treatment consent form, assignment of payment form, and the HIPPA form can all be filled out prior to arriving for your first visit in order to save you time.

Patient History

Once the paperwork has been completed, you will have a consultation with the chiropractor to discuss your health-related problems, concerns, as well as potential treatment options.

History and Examination

In order to determine what your actual problem is, the Dr. Mroski will ask you various questions related to your condition. Next, a number of specialized tests will be performed to determine which tissues are affected and to what extent. The history form is available here so that you will have a chance to fill it out at your convenience prior to you initial visit if you wish. Please fill only the first page down to the dotted line and do not complete the reverse side.

X-ray Studies

Your specific condition may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you.

Depending on the complexity of your individual case, the doctor may elect to start treatment during your first visit. All patients are different, though, and he may decide that he needs to wait until your next visit to start treatments so that he can collect and review all of the information necessary to give you the best care possible.

Home Instructions

Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.

Schedule Your Next Appointment

You will be given a convenient appointment time for your follow-up visit. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of findings. Treatment options for your condition will also be discussed during this time.

We hope that this information will make your first visit at Nothshore Chiropractic an enjoyable experience.
